Enrolment Form

All classes start with you filling out an enrolment form. 

Please download the attached form here, fill it out and return it to the office to secure your enrolment.

You will notice that the form cannot be filled in online as we require your signature. Once downloaded

you can print it out and return it directly to the community house or send it as a scanned document to office@hamptonparkch.vic.edu.au
